Case Management Society of America (CMSA)
Administrative Case Management (ACM) delivers short term, office based support for children receiving mental health services from multiple providers.
Blended Case Management (BCM) gives priority to children who are at risk for out of home placement and receive mental health services from multiple providers. BCM offers community based support focused on resilience and wellness. The nature of the program is holistic and consumer driven with most services delivered in the community.
Eligibility for Montgomery County Case Management for Children: Children and adolescents, up to and including 18 years of age, and their families can be referred by family, school, behavioral health provider or a social service agency. Children must reside in Montgomery County.

Services offered to children by Creative Health staff are:
Child Centered as they meet the individual needs of the child, considers the child’s and family’s situation, are developmentally appropriate, strength based and child specific.
Family Focused as the family is recognized as the primary support system for the child. The family is a full partner in all stages of decision making and treatment.
Community Based as both formal and informal resources are accessed in order to promote the child’s successful participation in the community.
Multi-Systemic as they are planned in collaboration with all child-serving systems involved with the child’s life.
Culturally Competent as the behavior, attitudes, values, beliefs, customs, language, rituals, ceremonies and practices of the child’s ethnic group are recognized and respected.
Least Restrictive/Least Intrusive as they are delivered in settings considered the least restrictive and least intrusive and meet the needs of the child.
